DISEASES OF THE HIP-PELVIC SYSTEM

Abstract

 Osteoporosis (OP) is typical for RA - both local (periarticular) and generalized. Periarticular OP is a well-known early manifestation of RA, developing before the formation of bone erosions, and one of the early radiographic diagnostic signs of the disease [A.N. Medvedev, 1996; E.L. Nasonov, 1997].

Today, there is quite a lot of information about the mechanisms of OP in patients with RA at the advanced and late stages of the disease. It has been established that the chronicity of inflammation and the progression of OP in RA are based on mechanisms associated with an imbalance between the production of

"pro-inflammatory" ("pro-osteoporotic") and "anti-inflammatory" ("anti-osteoporotic") cytokines [A.K. Kasumova, A.M. Satybaldyev, A.V. Smirnov, 2004]. It is currently known that cytokines exert their influence on the bone remodeling process through the RANKURANK/OPG system. In bone tissue, interleukin-6 (IL-6) stimulates osteoclastic formation by indirectly activating RANKL expression [N.N. Kartamysheva, O.V. Chumakova, 2004; S.A. Ketlinskii, A.S. Simbirtsev, 2008; Giiler-Yйksel M. et al., 2008].

Interleukin-1 (IL-1)romotes cartilage tissue degradation by enhancing the synthesis of collagenase, neutral proteases and metalloproteinases and increasing RANKL production by stromal cells and osteoblasts [N.N. Kartamysheva, O.V. Chumakova, 2004; S.A. Ketlinsky, A.S. Simbirtsev, 2008] by binding it to specific receptors, in particular, to the type 11 receptor (IL-IRI!). However, no studies have been found describing the dynamics of the content of (L-lRII in patients with RA at an early stage.
